Are you a new patient? Here is important information you need to know about becoming a patient in our clinic.

The Eleanor and Lou Gehrig ALS Center at Columbia University operates two, full-day multidisciplinary ALS clinics each week. We have also recently established a monthly satellite clinic at Columbia Doctors Tarrytown to serve better our ALS patients and families in Rockland and Westchester counties, Fairfield county, northern New Jersey and beyond.  

Make an Appointment

To make an initial appointment with an ALS physician, please call 212-305-6788, or email Neuro_ALSCenter@cumc.columbia.edu. All relevant medical records should be faxed to 212-305-1504.

Diagnostic Consultation

An initial evaluation by a Columbia University neurologist at the New York Neurological Institute is required to confirm a diagnosis of ALS/MND in all patients referred to the Eleanor and Lou Gehrig ALS Center. If appropriate, you will be referred to the ALS Clinic for further evaluation, education, and management.

First Clinic Visit

At the first ALS Clinic visit, each patient meets with an ALS physician and our nurse practitioner, who oversees our multidisciplinary care team. Working together, we assess each patient, determine an individualized plan of care, and implement that plan after the first clinic visit.
